Description

• Definition: From the Latin affidavit, meaning "I declare under oath". It is a sworn document that serves as testimony before an agency, entity or court, whether public or private. It is synonymous with affidavit.
• If you are outside of Puerto Rico, you can usually go to a local notary. However, we recommend that you first check with the entity in Puerto Rico that is requesting the document, in case there are any additional requirements.

What should an Afidávit include?

• The facts stated must be true to the best of the declarant's knowledge. Giving false information constitutes perjury, a crime.
• The notary must verify the identity of the declarant by:
  • valid identification, such as a driver's license or passport; or
  • personal knowledge of the notary about the declarant.

• If you need to use the affidavit outside of Puerto Rico and the United States, you may need an Apostille or certification for international use, depending on the destination country.
